How many practitioners?

We have two providers and one collaborating physician. Chris Donzell, FNP-BC, Ashley Rozeveld FNP-BC, and collaborating physician Gregory Lambourne.


What is the scope of practice?


According to the American Association of Nurse Practitioners, nurse practitioners (NPs) are licensed, independent practitioners who practice in ambulatory, acute and long-term care as primary and/or specialty care providers. NPs assess, diagnose, treat and manage acute episodic and chronic illnesses. NPs are experts in health promotion and disease prevention. They order, conduct, supervise, and interpret diagnostic and laboratory tests; prescribe pharmacological agents and non-pharmacologic therapies; and teach and counsel patients, among other services.


Is a Registered Nurse that can facilitate self-determination, self-care, comprehensive, holistic approach, link with community resources, and assist with navigating the health care system.

Chronic care management (CCM) services are available if one has increased health risk and has been diagnosed with two (2) or more chronic conditions which are expected to last at least twelve (12) months.

How does it work? Our goal is to empower the patient to take charge of his or her own health and become actively involved in improving your quality of life.

Our team is focused to make sure you get the best care and support possible on an on-going basis. We will call you to:

  • check your health conditions

  • refill your medications

  • change or prescribe new medications

  • guide you how to get better in health

You will have access to our team:

  • during clinical and off-clinic hours

  • help you transition in/out of the hospital

  • arrange follow up care

  • provide resources for more support
